CD specific programs
- Concurrent Disorder Partnership Program (ReCharge): The Concurrent Disorder Partnership Program is a partnership between Reconnect, Canadian Mental Health Association-Toronto and Humber River Regional Hospital and offers adults, group and individual support to understand and accept their illness. The program is currently only available to clients of the partner agencies.
All other programs
- Assertive Community Treatment ( ACTT ) (age 16+ years, West Toronto (north of Eglinton): ACTT is an intensive community-based mental health service provided by a multidisciplinary team for people with severe and persistent mental illness and complex needs.
- Community Support Program (case management) (age 16+ years, West Toronto): The Community Support Program (case management) helps people reach their recovery goals and live successfully in their community of choice. Community Support provides intensive individual assistance in accessing community resources and services.
- Case Management for Seniors (age 60+ years, West Toronto): The Community Support for Seniors Program (case management) helps older adults reach their recovery goals and live successfully in their community of choice. The program provides intensive community- based support for older women and men.
- Dufferin Program (case management) (16+ years, Dufferin County): The Dufferin Short-Term Community Support Program (Case Management) helps people reach their recovery goals and live successfully in their community of choice. The program provides short-term intensive individual assistance in accessing community resources and services.
- Mental Health and Justice Prevention Program (age 16+ years, West Toronto): The Prevention Program serves women and men with a mental illness who are (or are likely to become) involved with the criminal justice system. The program provides responsive short-term case management and helps individuals reduce or prevent involvement with the criminal justice system. Our program is part of a city-wide initiative. We work with the following agencies to offer this program across Toronto: Canadian Mental Health Association - Toronto, COTA Health, Sound Times, Fred Victor Centre, Toronto Friendship Centre and Central Toronto Youth Services.
- Rehabilitation Day Program (Social Rehabilitation/ Recreation) (age 16+ years, Greater Toronto Area): The Rehabilitation Day Program provides daytime group programming and individual support focused on achieving client goals. Clients work towards recovery through group participation and individual support.
- Dual Diagnosis Day Program (Social Rehabilitation/Recreation) (age 16+ years, West Toronto): The Dual Diagnosis Day Program is a one-year program that offers group programming and case management to individuals who have a dual diagnosis (serious mental illness and developmental disability). Through the program, participants are assisted in transitioning to long-term supports in other Reconnect programs or in other community-based programs. The program is offered in partnership with Griffin Centre and CORE.
- Mental Health and Justice Housing Support (Support Within Housing) (age 16+ years, West Toronto): The Mental Health & Justice Housing Support Program provides permanent housing, support to maintain housing, and help to prevent future involvement with the criminal justice system for individuals who have a serious mental illness, have current involvement with the criminal justice system and are homeless. The program is part of a larger initiative administered by four lead agencies: Loft, COTA Health, Houselink and the Canadian Mental Health Association -Toronto (CMHA). Reconnect and CMHA work in partnership to provide housing support and housing, respectively.
- Supportive Housing Etobicoke/York (Support Within Housing) (age 16+ years; Etobicoke and York): Supportive Housing Etobicoke York offers permanent housing and housing support to adults who have a serious mental illness, are homeless or at risk of homelessness, through a partnership among Reconnect, the Trillium Health Centre and Supportive Housing in Peel (SHIP). Units are accessed by SHIP and housing support (case management) is offered by Reconnect and Trillium Health Centre.
- Housing and Support Peel (HASP) (support within housing) (age 16+) ( Brampton and Malton): The HASP program offers permanent housing and housing support through a partnership to individuals who have a serious mental illness who are homeless or at imminent risk of homelessness. Housing units are accessed by SHIP and housing support (case management) is offered by Reconnect and the other partner agencies.
- Short-Term Residential Bed Program (age 16+ years, West Toronto): The Short-Term Residential Bed Program provides temporary (up to 30 days) residential support in the community to individuals with severe and persistent mental illness, who are in crisis, have current involvement with the criminal justice system and are homeless. The goal of the program is to assist the individual in accessing long-term support and to help prevent future criminal justice involvement. Our program is part of a larger initiative. Our partners, Canadian Mental Health Association - Toronto (CMHA), COTA Health and the Gerstein Centre, each provide similar services.
- Social Resource Centre (age 16+ years) (West Toronto): The Social Resource Centre is a program offered through a partnership between Reconnect and the Canadian Mental Health Association - Toronto (CMHA). The Social Resource Centre offers daily programming which provides opportunities for social and peer support as well as leadership and skill development. This program is available to clients of Reconnect and CMHA.
- Streets to Homes (age 16+) (West Toronto): The Street to Homes program (a program of the City of Toronto) provides housing and support to individuals who are homeless. Through a partnership with the City of Toronto, Toronto North Support Services and COTA Health, intensive follow-up supports are provided to help ensure housing stability. Reconnect receives referral from the City of Toronto.
- Transitional Case Management : The Transitional Case Management (TCM) Program provides support to individuals who have a serious mental illness and are involved with the criminal justice system. The goals of the TCM program are to facilitate a successful transition from hospital to community and prevent future criminal justice involvement for individuals who are under the jurisdiction of The Ontario Review Board (ORB). The program is a partnership between Reconnect, Canadian Mental Health Association - Toronto (CMHA) and the Centre for Addiction and Mental Health (CAMH) Law and Mental Health Program. Only referrals for CAMH are accepted.
- Youth: Community Choices for Success : Youth: Community Choices for Success (YCCS) offers group and individual support to women and men age 18 to 30, to assist them in reaching their employment and recovery goals. This comprehensive program is provided through a partnership between Reconnect and JVS Toronto and is specifically geared to the needs of those with a dual diagnosis (mental illness and developmental disability).

Reconnect Mental Health Services is a community mental health agency that provides a range of recovery-based programs for adults experiencing serious mental illness. Inspired by hope in the individual's potential to recover, Reconnect works collaboratively with adults experiencing serious mental illness to reach their goals and improve their quality of life. Reconnect began in 1981 as a project to serve discharged psychiatric patients in Etobicoke (especially after the closing of the Lakeshore Psychiatric Hospital in 1979). The current Board of Directors includes consumers, family members, mental health and business professionals, and other interested community members. A Client Council composed of consumer representatives provides input and direction to the Board, staff and clients.
Over the years, Reconnect has expanded significantly. We are proud of the services we provide and pleased to offer a range of diverse community based programs developed to meet each client's individual needs.
